Twenty cents. That's what a used syringe is worth in New York City now. Bring in fifty a day and you walk out with cold hard cash — no questions asked about where those needles came from or what was in them.
Mayor Zohran Mamdani just locked $3 million into the city's $125.8 billion budget to make this arrangement permanent.
The program launched as a pilot in March 2025 under Local Law 124, which Council Deputy Speaker Diana Ayala authored back in 2022. The city hired a Boston nonprofit called Addiction Response Resources on an $11.1 million contract to run the operation. In its first year, 1,700 participants collected $292,000 in total payouts — one syringe at a time, twenty cents a pop, capped at $10 per day per person.

This wasn't a controversial program according to the city council members either. The budget line sailed through in a 45-6 vote. The program currently operates out of eight sites in the Bronx and one each in Brooklyn, Manhattan, and Queens. City officials say they've collected 2.3 million syringes since launch.
Health Department spokesperson Rachel Vick frames the whole thing as "harm reduction" — the idea being that paying addicts for their used needles keeps those needles off sidewalks and out of parks. But San Francisco already tried this program. Addicts figured out how to game the system to score more money for drugs in record time.
Councilwoman Joann Ariola, a Republican from Queens, wasn't buying it. She pointed out that paying active drug users cash — with zero requirement to seek treatment, attend counseling, or do anything other than show up with dirty needles — isn't harm reduction. It's subsidization.
She's got a point the numbers don't answer. The $292,000 paid out in year one went exclusively to people actively using intravenous drugs. Not a single dollar of that money was conditioned on entering rehab. Not one participant was required to speak with a counselor. The program's entire theory of change is: pay them, collect the sharps, repeat tomorrow.
Councilmember Oswald Feliz, a Democrat from the Bronx where eight of the eleven sites operate, praised the program as meeting people "where they are." That phrase does a lot of heavy lifting when "where they are" is shooting up daily and getting paid by the city for the privilege of not leaving their needles on the playground.
Mamdani's administration secured over $189 million in opioid settlement dollars for the city. Three million of that is now earmarked to ensure the cash-for-syringes pipeline runs indefinitely. The formal implementing rules were adopted in May 2024. The pilot is over. This is city infrastructure now.
Rehab beds. Counseling services. Job training. Supervised detox. The list of things $3 million could fund is long. Instead it buys used needles at twenty cents each from people the city has decided aren't worth the effort of actually helping.
That's not harm reduction. That's a subscription service.
